My work at Refactor Games
Trypophobia is a narrative driven psychological horror game centered around the fear of small holes. The planned run time is 1 - 2 hours. The key features are the powerful tools of Unreal Engine like Lumen and Nanite to create a highly realistic, immersive experience and the thought provoking, mysterious narrative that allows the player to put the pieces together themself.

I start with my experience goal for the environment. This could be as vague or as specific as needed but it should incorporate the narrative and mechanics of the game. Here the goal being "The player should feel tense as they explore the environment, prepared for something to happen at any time. When they are pursued, anxiety turns into terror as the monster catches up to them."
Now the idea for the environment is shaped around this: "The player navigates a network of claustrophobic tunnels and caverns that limit their line of sight".
And incorporates the mechanics (AI monster chase): "The environment features several straights where the player can run away from the monster, but will soon be caught. Obstacles, short cliffs in this instance, allow the player to break away if used carefully."
Now I sketch out my idea, including notes on key points (Usually more detailed but kept light here). Then finally greyboxing my idea in Unreal Engine, while keeping my goals in mind. Adjustments were made along the way but I'm very happy with the result!
